Confidentiality Agreement

To ensure privacy protection as part of the Health Insurance Portability and Accountability Act (HIPAA) and to provide individuals with control over what personal information is used & disclosed, I agree to provide the above information to Colorado Ovarian Cancer Alliance. COCA keeps all info confidential and does not share it with outside organizations.

Disclosure and Informed Consent

The Colorado Ovarian Cancer Alliance (“COCA”) is the host organization for all Nicki’s Circle Support Groups (both monthly and weekly) for survivors of ovarian and gynecologic Cancers and caregivers. Please carefully read the following statements about Nicki’s Circle and confirm your acknowledgment of reading and agreeing to each statement by electronically signing at the end of the document. 

 

Nicki’s Circle (NC) is a free support group that offers a safe and caring environment for survivors of gynecologic cancers and caregivers. All groups are facilitated by master level counseling students and or Licensed Professional Counselors. The following are potential reasons to attend Nicki’s Circle - meet with others who are experiencing similar challenge, explore and share feelings about your situation, discuss problems and stress arising from OC and or caregiving, learn about resources, treatment options, and hear different w to manage side effects. 

 

Recognizing the unique challenges of gynecological cancers, Nicki’s Circle Survivor Support Groups are open to gynecologic cancer survivors ONLY.
 
Recognizing the unique experience of providing care to individuals diagnosed with gynecological cancer, Nicki’s Circle Caregiver Support group is open to caregivers ONLY – including but not limited to spouses, siblings, adult children, friends, and family members of gynecologic cancer survivors.

 

TECHNOLOGY
There are potential risks to videoconferencing technology, including interruptions and unauthorized access. COCA will communicate with you exclusively through one or more of these channels: a @colo-ovariancancer.org email address, Zoom videoconference, Vastconference, Google meets, or by phone.

 

RESOURCES
The information shared in Nicki's Circle Support Groups does not reflect the opinion of, or reflect endorsement by the Colorado Ovarian Cancer Alliance. Nicki's Circle Support Group provides an opportunity for members to share their own personal experiences, opinions, and concerns so that they may receive information and support from other members. Materials and resources expressed during group are not intended to diagnose, treat or cure, and should never substitute for regular medical care.  It is advised to never change your protocol based on any shared resources during group, or based on what another member is doing, without consulting with your doctor first. 

 

A SAFE ENVIRONMENT   
A safe environment is created and maintained by both the facilitator(s) of a group and its members. A list of group guidelines will be read and given at each meeting, of which, it is non-negotiable for each member to observe in order to attend Nicki’s Circle. Another non-negotiable observance for participation and a safe environment is maintaining confidentiality. Your group facilitator(s) are bound by law to maintain confidentiality, as group members are bound by honor to keep confidence. We realize that you may want to share your particular insights about yourself during group with a significant other. This is fine as long as you remember not to talk about how events unfold during group or in any way that might compromise the confidentiality of other group members.  

 

Attending support groups can have benefits and risks. In group individuals often discuss unpleasant aspects of life causing you to experience uncomfortable feelings like sadness, guilt, anger, frustration, loneliness, and helplessness. On the other hand, attending support a group has also been shown to have benefits for people who go through it. Attending support groups often leads to better relationships, solutions to specific problems, and significant reductions in feelings of distress. But there are no guarantees of what you will experience.

 

INFORMED CONSENT
You may withhold or withdraw this consent at any time by providing COCA with written notice. Otherwise, this consent will be considered renewed and ongoing upon each meeting attendance. In consideration for your participation in the Nicki’s Circle program, you agree to indemnify, defend, release and hold harmless COCA and Nicki’s Circle, their employees, officers, directors, agents, successors and assigns, against all liabilities, claims, causes of action, damages, judgments, costs or expenses, including attorney's fees, caused by, resulting from or which arise in connection with your participation in the Nicki’s Circle Support Group program and all supportive services.

Follow-up

Please Note:
The Support Group Facilitator will be following up with you by phone before you attend for the first time to provide additional information and answer any questions you may have about the group. She will contact you by email to set up a time to talk.
