Support the ministries of Quaker Lake Camp by

participating in the 2020 QLC Golf Tournament.

Complete this form to sponsor, register a golf team, and/or make a donation.

 

This year, to allow for physical distancing for the health and safety of participants,

tee times will be assigned and teams will play throughout the day.

Golfers must register and pay by July 3 to reserve a tee time.

No payments will be accepted at the tournament.

Winners will be contacted and announced on our website and social media accounts.

Each golfer will receive a goodie bag with snacks, and lunch will be provided.

Golf Team

Check the box below to register a team of four in the tournament.

  • In the "Additional Comments" box at the bottom of this form, indicate your team name, and the names, cell phone numbers, and email addresses for the other three golfers on your team.
  • Also in the comments box, indicate your top 4 preferences for tee time (7:30 a.m. - 2:45 p.m., tee times every 15 minutes).
  • Select the appropriate amount under "Gift Information" below to process your electronic payment. If you wish to purchase mulligans (maximum 2 per player) for yourself or the team, add $5 for 1 mulligan, $10 for 2, or $30 for the team and enter the total amount you're paying in the box for "Other."
Golf Team Registration - $240

Sponsorship/Donation

Select an option below under “Sponsorship Level” to sponsor the Quaker Lake Camp Golf Tournament or make a financial contribution. You also will need to select the appropriate amount under "Donation  Amount" below to process your electronic payment.

 

Hole Sponsor - $100

Each Hole Sponsor has a sign on the course the day of the tournament. In the comments block below, indicate you would like to use a sign from a previous year or provide the text as it should appear on the sign.

 

Corporate Sponsor (with team) - $1,200

Corporate Sponsors (with team) receive recognition on the QLC website and signage at the event. If commitment is made by June 20, donor also may enter a team of four in the tournament. In the "Additional Comments" box at the bottom of this form, indicate the name as it should appear in recognition and your team name, as well as the names, cell numbers, and email addresses of the other three players on your team. Also indicate your top 4 preferences for tee time (7:30 a.m. - 2:45 p.m., tee times every 15 minutes). Select the appropriate amount under "Gift Information" below to process your electronic payment. Corporate Sponsors receive recognition on the QLC website and signage at the event. In the comments block below, indicate the name as it should appear in recognition.

A financial contribution in any amount is appreciated. Indicate the amount you wish to contribute under “Donation Amount” below. If you also are registering a team or sponsoring the event, add the amount of your donation plus the cost of the team or sponsorship and enter the total under “Donation Amount” below.
